At Home is a medical model providing essential services that empower community members to remain in their homes as long as they are safe.  Assured  services include rides to medical appointments, grocery shopping, twice-monthly home visits from a licensed healthcare professional, home safety assessment, coordination of services, and much more. At Home’s staff and volunteers follow all CDC guidelines when working directly with members, ensuring their safety.

To receive services, members pay an annual sliding scale fee based on their annual income. At Home currently offers services on the Blue Hill Peninsula and Mount Desert Island.
